Notice for LPG connection

This is to inform you that a special initiative has been taken by the District Administration to upkeep the cooking method of Cooked Mid-Day-Meal Programme (CMDMP) in more modernized & Hygienic way by providing LPG connection to 100% intended schools under this district within December, 2018.

As a part of the initiative a meeting was conducted at district level through along with various Officials / representatives of dealers association of three company i.e. Iindian Oil Corporation Limited (LOCL), Hindustan Petroleum (HP) and Bharat Petroleum (BP) on 28.10.2018 where it has been resolved that for quick and hassle free LPG Connection, the following documents are to be submitted to nearby LPG Company dealers.

1. Filled in Application from for obtaining LPG connection (IOCL, HP  & BP) as per Annexure-A.

2. Filled in Fire & Safety Declaration as per Annexure-B.

3. Necessary KYC document (U-DISE copy/ Bank Details/ Electric Bill etc.)

A summarized guideline in this regards is attached herewith as Annexure-D for ready reference.

In view of above, you are directed to circulate all sets of documents to all schools where LPG connection does not exists by 13-th October, 2018 positively with an instruction to submit the application from with all required documents to respective Block/ Municipality latest by 15th November, 2018 without fail. The Block Development Officers / Chairman of Municipalities are also requested to recommend and submit the same to the respective LPG distributor/ dealers and a synoptic report in Annexure-C (in MS-Excel Format) should be submitted to this end immediately after completion, on which necessary funds will be calculated and sub-allotted accordingly.
