Enclosed please find photocopy of the an Order vide Memo No. 106-Secy(H&FW)/2021 dated 18.05.2021 received by this office from the Secretary Health and Family Welfare Department regarding "Guidelines for Covid Vaccination" which is self-explicit.

In view of the above you are requested to send the format-wise data base of the teacher to this office for Covid-19 vaccination.

Vaccination of all Teaching and Non-teaching staff of Institutions

I am directed by the District Administration to inform all the Head of the Institution (HOI) of Junior/ Secondary High/ Higher Secondary Schools and Madrasahs of Murshidabad District that a database is being made for Vaccination of Covid in favour of all Teaching and Non-teaching staff of the Institutions. Two Google Forms for each Sub-Division are attached herein. One is for above 45 years and another one is meant for below 45 years of age. Already 2 doses vaccinated staffs name need not require to get entry in this database.

Please submit the report of your Institution by 21.05.2021, 1:00 pm without fail.

In terms of issuance of Government order vide Memo No. 106-Secy(H&FW)/2021 dated 18.05.2021 and as per direction of the higher authorities towards preparing district database for COVID-19 Vaccination Programme the following officers and staff of the office of the District Inspector of Schools (SE), C/O Additional District Inspector of Schools (SE), Durgapur Sub-Division are to be entrusted with the duly of Monitoring-cum-Reporting officer for circles mentioned against his/her name. The Sub-Inspector of Schools of concerned circles will act as Nodal Officer for preparing the same at Circle Level.

Vaccination to Teachers/ Instructors of VTC

Urgent notice for furnishing Corona Vaccination Status of Teachers/ Instructors for Grantee VTCs

This is to inform Head of the Institution (HOI) of all Grantee running VTCs to update Corona Vaccination Status of Teachers/ Instructors in i-OSMS Portal as mentioned below:

The Head of the Institution (HOI) will login the portal with their password. Then a popup from will be opened with respect to updation of Corona Vaccination Status of Teachers/ Instructors of 1st VTC. Drop down menu will be there for updation. After correct entry of status as mentioned above for all Teachers/ Instructors, the same must be submitted. The portal will be opened for this purpose from 25.05.2021 to 27.05.2021.

Again, it is reiterated that Head of the Institution (HOI) of any Grantee VCT should not ignore it to avoid any dispute. 

No communication other than the above procedure in i-OSMS Portal is allowed as that information could not be included in i-OSMS Portal by Directorate/ Council/ Nodal Polytechnic.
