Notice

Candidates requiring Medical Fitness Certificate will need to attend Eye, ENT, Surgery, Gynecology (for females and Cardiology OPDs with CBC, FBS, serum Creatinine, Urine RE and ME, Chest X-ray (PA View) and ECG 12 leads, before attending Medicine OPD for final clinical assessment and fitness certification, once the above are all in place.

Candidates requiring opinion of Medical Board will apply to the Office of the MSVP for constitution of medical board and date of appearance, with the above medical reports and past medical documents if any.

Class IX-X and XI-XII

Medical Examination for Assistant Teacher of Northern Region Class IX-X and XI-XII under North Bengal Medical College and Hospital, Siliguri, West Bengal.

Service Confirmation এর জন্য নবনিযুক্ত Teaching and Non-teaching Staff যাদের 2-বছর Service সম্পূর্ন হয়েছে তাদের Medical Fitness Examination বা Medical Verification প্রক্রিয়া অনুষ্ঠিত হবে। নিম্নে তার বিস্তারে আলোচনা করা হলো :

সংশ্লিষ্ট জেলার হাসপাতাল (North Bengal Medical College and Hospital) নির্দেশিকা মতো Teacher-দেরকে Schedule Date and Time অনুযায়ী সমস্ত Medical Fitness Report, নিজের Documents ও Medical Fees সহ ঐ হাসপাতালের Medical Board এর সামনে উপস্থিতি হতে হবে।

Necessary documents and Medical Fees

(a) Approval, Appointment Letter, Recommendation Letter and Joining Report (Original & Xerox)
(b) PH Certificate (Original & Xerox)
(c) Xerox of EPIC/Aadhaar Card/ PAN for ID
(d) School Certificate ( To Whom...) from Head of the Institution (HOI)
(e) Two copies of recent Passport size Photos
(f) Cash Rupees 75/- ( আরো Rs. 500/- টাকা দিতে হতে পারে)
(g) 4-Copies of bank deposited Challan Form /Online e-Challan Form

প্রথমে Mobile বা Computer থেকে একটা Rs. 25/- (Twenty five only) চালান কাটতে হবে। 

সর্বপ্রথমে wbifms.gov.in-তে যেতে হবে। তারপরে 'Health & Family Welfare' হেডে Challan কাটতে হবে। 'Depositor' এর নাম, অন্যান্য Point গুলো Fill-up করতে হবে। এক্ষেত্রে Sub-head হবে 'Miscellaneous Service' এরপর Comment থাকলে দিতে হবে।

Medical Fitness Examination or Medical Verification from Hospital


(i) Refraction Report of both eyes (Colour vision)
(ii) Report of X-Ray Chest (P.A View) with place
(iii) E.C.G
(iv) Routine Examination of Stool, Urine, Albumin S.G
(v) Blood for TC, DC, ESR, Hb %, Blood Sugar
(vi) Pure Tone Audiometry (PTA) Report

Ticket for OPD
সকাল 9:00 টার মধ্যে Medical College-এ পৌঁছাতে হবে। তারপর আউটডোরে দেখানোর জন্য Rs. 2/- টাকার OPD Patient Card (টিকিট) কাটতে হবে। দেখাতে হবে Medicine Department এ। ইচ্ছা করলে একাধিক টিকিট কেটে নিতে পারেন পরে কাজে লাগতে পারে। টিকিট কাটার জন্য EPIC/Aadhaar Card /PAN ব্যবহার করুন, এতে নামের বানান ভুল হবে না।

Medical Test for EYE, ENT, Surgery and Gynecology Department

টিকিট কাটা হয়ে গেলে 18 নম্বর রুমে যেতে হবে, সেখানে টিকিট রেজিস্টার করা হবে। এরপরে Medical Test এর জন্য ডাক্তার দেখাতে হবে। ডাক্তারবাবু EYE, ENT আর Surgery Department এ রেফার করবে আর কিছু Test লিখবেন, যেমন-Female Candidate দের ক্ষেত্রে Gynecology Department.

Eye এর জন্য EYE Department, Room Number 23, একইভাবে বাইরে টিকিট রেজিস্ট্রেশন করাতে হবে। চোখ দেখানোর জন্য লাইনে দাঁড়াতে হবে।Room No. 23 এর এক পাশে সিনিয়র ডাক্তারবাবুর কাছে গিয়ে প্রথম চোখের চেকআপ রিপোর্ট দেখে Certify করতে হবে। এরপর রেজিস্ট্রেশনের ওখানে এসে স্ট্যাম্প দিয়ে নিতে হবে।

Next ENT Department, Room Number 24, প্রথমে টিকিট রেজিস্ট্রেশন করাতে হবে।ডাক্তার দেখানোর জন্য লাইনে দাঁড়াতে হবে।ডাক্তারবাবু  PTA Test জন্য লিখবেন।পাশে অডিওমেট্রি রুমের লাইনে দাঁড়িয়ে PTA Test করাতে হবে। Test হয়ে গেলে ডাক্তারবাবুর কাছে এসে অডিওমেট্রি রিপোর্ট দেখিয়ে Certify করে নিতে হবে।যেখানে টিকিট রেজিস্ট্রেশন করানো হয় সেখানে এসে স্ট্যাম্প দিয়ে নিতে হবে।

Surgery Department, Room Number 7, প্রথমে টোকেন রেজিস্ট্রেশন করাতে হবে। ডাক্তার দেখানোর জন্য লাইনে দাঁড়াতে হবে।ডাক্তারবাবু চেক করে Certify করে দেবেন।যেখানে টিকিট রেজিস্ট্রেশন করানো হয় সেখানে এসে স্ট্যাম্প দিয়ে নিতে হবে।

Female Candidate দের ক্ষেত্রে Gynecology Department যেতে হবে। প্রথমে টিকিট রেজিস্ট্রেশন করাতে হবে। ডাক্তার দেখানোর জন্য লাইনে দাঁড়াতে হবে।ডাক্তারবাবু চেক করে Certify করে দেবেন।যেখানে টিকিট রেজিস্ট্রেশন করানো হয় সেখানে এসে স্ট্যাম্প দিয়ে নিতে হবে।

Test reports for Medical Fitness Examination

এই Test গুলো বাইরের কোন বেসরকারি Pathology বা Lab থেকেও করা যেতে পারে, এতে আপনাদের হয়রানি ও সময় দুটোই বাঁচবে।

সমস্ত Test Report গুলো হাতে পাওয়ার পর Test Reports আর টিকিট নিয়ে মেডিসিনের ডাক্তারের কাছে যাতে হবে।সমস্ত Test Reports এবং উপরের 3/4-টি  Department (EYE, ENT, Surgery and Gynecology) এর রিপোর্টগুলো দেখে ডাক্তারবাবু Final Certificate দেবেন।ডাক্তারবাবু টিকিটের মধ্যেই Certificate দেবেন।এই ক্ষেত্রে একাধিক টিকিট প্রয়োজন হতে পরে। তাই হয়রানি এড়াতে প্রথমেই দুটি টিকিট কাটা উচিত।

বি: দ্রঃ Report বা Test এ কোন Problem দেখাদিলে সেই Candidate কে অন্য ডাক্তারের কাছে রেফার করবেন বা আরো কিছু Test করতে বলবেন।

Submission Certificate and Documents

এবার সমস্ত কাগজ এক কপি Xerox করে সুপারের ঘরে জমা করতে হবে। Documents জমা দেওয়ার পর Certificate দেওয়ার সময় Rs.75/- টাকা সংগ্রহ করে নেবেন।সুপারের ঘরে কি কি জমা করতে হবে -

(a) Ticket (Final Certificate), Radiometric Report and Test Reports
